While sinus pause is frequently a benign finding in adults, commonly occurring during sleep, sometimes it can be abnormal. We present a case of symptomatic sinus pause in the setting of increasing doses of ropinirole. To our knowledge, this is the second case reported in the literature.

21 year old white Caucasian male presented with left foot pain and bilateral periorbital hypopigmentation. An MRI revealed multiple stress fractures. Blood tests revealed Vitamin D deficiency. There is currently evidence confirming relationships between Vitamin D deficiency and cutaneous hypopigmentation. This case suggests further research into this area.

Esophageal perforation is a rare but life-threatening condition1. Early detection and diagnosis are important to ensure good patient outcomes. The case that we present is of a patient that developed spontaneous esophageal perforation following irrigation and drainage of a retropharyngeal abscess.

A 33-year-old male admitted with sudden and spontaneous painless left infraorbital swellings after forceful nose-blowing. He was diagnosed orbital emphysema with orbital floor blow-out fracture. This case shows that forceful nose-blowing can cause orbital floor fracture and subcutaneous orbital emphysema by rapid increase in pressure in the upper airways.

Differential diagnoses of patients with multiple bony lesions that are confined to one side of the body should include polyostotic fibrous dysplasia with and without McCune-Albright Syndrome, particularly if there is no history of malignancy. Lesion biopsy ultimately provides the definitive diagnosis.

An 88-year-old lady presented with idiopathic sepsis and ischaemic gangrene of the right breast. Group-A bacteraemia was identified on blood culture. Breast biopsies taken were negative for malignancy. The patient was treated with intravenous antibiotics and fluid resuscitation. Subsequent wound care and continuation of oral antibiotics led to complete recovery.

Paraganglioma is a benign neuro-endocrine neoplasm rarely localized on the lumbar spine. A 50-year-old male who presented for paraparesis and urinary leakage. Lumbar spine MRI showed a lesion compressing the dural sac from L1 to L5. The patient was operated, and the pathologic examination concluded to a cauda equine paraganglioma
